Overview

This section covers the medication related data models available in iPCV.

The Medication data models represent the lifecycle of a prescription from the point at which a clinician prescribes a medication through to each individual issuing event and related care records linked to an ongoing problem.

When a healthcare professional prescribes a medication for a patient, a Drug Record is created. This record represents the prescription itself and contains the core details of the medication, such as the drug prescribed, dosage instructions, quantity, and prescription status. The Drug Record can be considered the “parent” record for a medication course.

Once the prescription exists, one or more Issue Records may be generated against that Drug Record. An Issue Record represents a specific instance of the medication being issued to the patient. This may include the initial issue created when the medication is first prescribed, if applicable, as well as any subsequent repeat prescriptions or reissues. As a result, a single Drug Record can be associated with multiple Issue Records over its lifetime.

From a data modelling perspective, a one-to-many relationship typically exists between Drug Records and Issue Records. Together, the two models provide a complete view of both the prescribed medication and its associated issuing activity.

Users should generally analyse both models together when investigating prescribing activity, medication usage patterns, repeat prescriptions, or patient medication histories, as each model provides a different perspective on the medication lifecycle.

Issue RecordMedication Issue record contains the transactional history of issuing activity for a given prescription in a patient’s record.
Problem Drug RecordBridge model linking problems or conditions to the Drug Records prescribed to manage them. Used to associate medication prescriptions with the clinical conditions they are intended to treat.
Problem Issue RecordBridge model linking patient problems or conditions to individual medication Issues related to a prescription to manage them. Used to analyse medication issuing activity and repeat prescribing against specific clinical conditions.
